An employee was working on a high pressure line. The fitting gave way and the line blew off striking his right ring finger causing two fractures and a laceration.

A truck driver with RJ's Transportation was unloading 93% sulfuric acid from the tanker truck to a holding tank at a customer site. Pressure lines failed and acid was released striking and burning the employee. Personal protective equipment (PPE) was not worn at the time.

An employee was performing maintenance on a CNC waterjet cutting machine by replacing the high pressure line. When he was testing the pressurized line, the fitting failed or broke causing the water to be released at a very high rate. It struck the employee in the chest and exited his back. The employee was standing approximately 15 ft. away from the machine.
